Other Forms

Hot Work Permit

• Required for all temporary operations involving open flames or producing heat and/or sparks.
• This includes, but is not limited to: brazing, cutting, grinding, soldering, thawing, and welding.

Crane Special Lift Form 

• Must be used when the weight of the load exceeds 75% of the crane's capacity, or 
• Must be used when the lift requires more than one crane. NOTE: No Yaskawa crane shall be used to lift personnel and no Yaskawa associate may be lifted by a crane.
